How thorough?
Don't type patient names, member IDs, claim numbers or dates of service. Claimvera masks them (and the names you list) before anything is sent, but it can miss things.
Claimvera is a review aid, not legal or medical advice.

AI keys
Paste a key, click Save, then Test. Claimvera switches that AI on straight away. No restart needed.
Keys are saved only on this computer, in Claimvera's .env file. They are never shown in full again and never sent anywhere except to that AI company. One key works; all three gives you reviewers from three different companies. Already using Octavera? Paste the same keys here.

Welcome to Claimvera
Thank you for being one of the first billers to use it.
How to use it
Work list: open the insurer's remittance file (the 835 from your clearinghouse or the payer's portal). Every denied line becomes a row with the denial in plain words, the recommended fix and the deadline. Or add a denial by hand from a paper letter.
Review a denial: open a row and run a review. Three AIs from three companies check the case and Claimvera writes the appeal, with every rule it cites looked up on the official site.
Appeal packet: one click builds the letter, a Medicare redetermination request when it applies, a cover sheet and the attachment checklist, filled in on your computer.
Your plan includes a number of AI reviews each month (shown on the denial page); they reset on the 1st. Reading files, rule checks and deadlines are unlimited.
Patient privacy
Patient details stay on your computer. Names, member IDs, claim numbers and dates from the remittance file are kept only in this browser. Claimvera and the AIs see codes, amounts, the payer and day counts.
Anything you type (notes, a question, a draft appeal) is masked before it's sent, but masking can miss things. Don't type patient details.
On a shared computer, use Forget patient details on this computer at the bottom of the work list when you're done.
Saved reviews are deleted automatically after 90 days. You can delete any review, or all of them, at any time.
Claimvera is a review aid, not legal or medical advice, and AI can be wrong. You remain responsible for what you bill and send.

Security & terms
The short version, in plain English. The full pages: Security · Terms
Where patient details go
Nowhere. The remittance file is read in your browser. Patient names, member and Medicare IDs, claim and account numbers and dates are kept in this browser on this computer, and are put into appeal letters only here.
What Claimvera receives for each denied line: the payer and payer type, procedure and diagnosis codes, modifiers, units, amounts, denial and remark codes, the service year and the number of days between service and denial.
What you type is masked first. Names, Medicare numbers (MBI), member IDs, claim and account numbers, record numbers, dates, addresses, phone numbers and ages over 89 become placeholders such as [MEMBER-ID-1]. Use Preview what's sent to see exactly what the AIs will see.
Three AI providers. The de-identified case goes to Anthropic (Claude), OpenAI (ChatGPT) and Google (Gemini) through their paid business APIs, which don't use it to train their models.
Rules are looked up on public official sites (Cornell's Legal Information Institute, the electronic CFR, ssa.gov and cms.gov). Only the rule number is sent to those sites.
Saved reviews (no patient details) are kept in your account for 90 days, then deleted automatically. Your billing company's admin can open reviews saved by people in the company. PODNalious staff don't open reviews.
Your responsibilities
Don't type patient details, and cover them in photos. Testers: use made-up or fully de-identified examples until your organization has approved Claimvera for real claims.
On a shared computer, forget patient details when you're done, and sign out. Each person gets their own login.
Check every letter, code and deadline before you send it.
Not legal or medical advice
Claimvera is a review aid for billing professionals. It is not legal or medical advice, and AI systems can be wrong. Scores are the reviewers' estimates, rule statuses describe what the official sites show, and calculated deadlines must be compared with the payer's notice and your contract. You remain responsible for what you bill and submit.

How Claimvera works
From a day of denials to appeal packets, without patient details leaving your computer.
Remittance file (835)→Patient details kept here→Rules checked by code→Three AIs review→Fix or appeal packet + deadline
The work list
Every denied line, sorted. Contract write-offs, deductibles, coinsurance and copays are left out. What's left is ranked by what it's worth: the amount at stake times the chance of getting paid, with deadlines on top.
The denial in plain words. Each adjustment and remark code is explained, with the group code (whether the patient can be billed).
The recommended path: corrected claim (for Medicare, a reopening), appeal, call the payer, bill the right payer, bill the patient, or write off.
Rules checked by code, not guessed
Official code lists: ICD-10-CM (valid and billable codes), HCPCS Level II, and Medicare's NCCI code-pair edits and unit limits, loaded from the files CMS publishes.
Built-in Medicare checks: therapy modifiers (GP, GO, GN), the chiropractic AT modifier, unlisted codes, unprocessable claims (MA130) that need a new claim instead of an appeal.
Medicare deadlines: redetermination 120 days, reconsideration 180 days, ALJ, Council and court 60 days each, counted from receipt (presumed 5 days after the notice), plus the 1-year reopening and 12-month timely-filing limits. Other payers use your payer profiles.
The AI review
Three reviewers from three companies: a coding and billing expert (Claude), the payer's own claims reviewer looking for why it would still be denied (ChatGPT), and a billing-company appeals specialist (Gemini).
Quick Check for routine denials, Deep Check (the default) for appeals, Full Reach for large or tricky ones.
Every rule cited is looked up: CFR sections, the Social Security Act, Medicare LCDs, billing articles, NCDs and Medicare manuals, each marked Verified, Not found or Couldn't check.
Which AIs
Currently Claude (Anthropic), ChatGPT (OpenAI) and Gemini (Google). Claimvera is independent and isn't endorsed by those companies, by CMS or by any payer.
Scores and chances are estimates, not guarantees. Claimvera is a review aid, not legal or medical advice.
Claimvera is built on PODNalious Technologies' adaptive multi-model orchestration (patent pending), the engine behind Octavera and Ledgervera.
