Turn a day of denials into a done list.

Open the insurer's remittance file. Claimvera sorts every denied claim by what it's worth, tells you whether to fix it or appeal it, writes the appeal with three AIs checking each other, and counts every deadline. Patient details never leave your computer.

For medical billing companies, clinics that do their own billing, and freelance billers. Invite-only early access.

The cumbersome part, done for you

Billers spend hours working out why a claim was denied, which rule applies and how to word the appeal before the deadline. Claimvera does the sorting, the looking up and the first draft.

The whole day's denials, ranked

Open the 835 remittance you already get. Contract write-offs and patient shares drop out; every real denial is ranked by amount at stake and chance of getting paid.

Fix it or appeal it, decided

Each denial in plain words, and one recommended path: corrected claim (a reopening, for Medicare), appeal, call the payer, bill the right payer, bill the patient, or write off.

The appeal packet in one click

The letter, a Medicare redetermination request with every required item, a cover sheet and the attachment checklist, with the patient's details filled in on your computer.

Deadlines that can't slip

Medicare's appeal levels are built in: 120 days to redetermination, 180 to reconsideration, 60 for each level after, counted from receipt. Other payers come from your payer profiles.

Rules checked by code

ICD-10-CM, HCPCS, Medicare's code-pair edits and unit limits are checked from the official CMS files. Therapy and chiropractic modifiers, unprocessable claims and unlisted codes are caught before you appeal.

Three AIs, checking each other

A coding expert, the payer's own claims reviewer and an appeals specialist, from three different AI companies. Every rule they cite (CFR, Social Security Act, LCDs, manuals) is looked up on the official site.

How it works

Built on the same multi-AI engine behind Octavera and Ledgervera, set up for medical billing.

  1. Open the remittanceThe 835 from your clearinghouse or the payer's portal, or add a paper denial by hand.
  2. Patient details stay putNames, IDs, claim numbers and dates are kept in your browser. Only codes and amounts go further.
  3. Rules by codeEach line is checked against the official code lists and Medicare's rules, and given a path and a deadline.
  4. Three AIs reviewFor the denials worth working, three AIs work the case and write the appeal argument.
  5. Send and trackPrint the packet, mark it sent, record the result. See what's being recovered each month.

A Quick Check takes about a minute; a Deep Check about two. Claimvera is a review aid and AI can be wrong, so the biller's judgment still decides.

Patient details stay on your computer

Claimvera was designed so it never needs to hold protected health information.

  • The remittance is read in your browser. Patient names, member and Medicare IDs, claim numbers and dates are kept in that browser and put into letters only there.
  • What Claimvera sees for each denied line: the payer, codes, modifiers, units, amounts, denial and remark codes, the service year and the number of days between service and denial.
  • Anything you type is masked before it's sent: names, IDs, dates, addresses, phone numbers and ages over 89 become placeholders.
  • No training on your data. Claimvera uses the paid business APIs of Anthropic, OpenAI and Google.
  • Forget on demand. One click removes every patient detail from a shared computer.

Questions

Is Claimvera HIPAA compliant?

Claimvera is designed so that patient details from the remittance file never reach our servers or the AI companies. We're completing an independent privacy review of that design. During early access, use made-up or fully de-identified examples unless your organization has approved Claimvera for real claims.

Does it replace my billing software or clearinghouse?

No. It works alongside them: it reads the 835 remittance you already receive and produces the fix, the appeal packet and the deadline. You still send corrected claims from your billing software.

Which payers does it handle?

Any payer's 835. Medicare Part A and B appeal levels and deadlines are built in. For Medicaid, Medicare Advantage and commercial payers, you enter each payer's appeal window and timely-filing limit once, and Claimvera uses it for every deadline.

Does it show CPT code descriptions?

Claimvera works with CPT code numbers. The official descriptions belong to the American Medical Association and need a license; your billing software already shows them.

Is this legal or medical advice?

No. Claimvera is a review aid for billing professionals. It shows what the codes and rules say and drafts the appeal; you decide what to send.
